Inspiration: Lack of comprehensive, curated information about skilled nursing facilities available from a single source. Varied and disparate sources of information compounded an already difficult decision for families. Discharge and case managers did not have access to a reliable source to assist them in selecting a skilled nursing facility, nor one to which they could direct patients.


Solution: The Koazie™ platform makes locating and selecting a post-acute care facility simpler, easier, and faster. The user-friendly website is a comprehensive, curated single source for information, streamlining choosing a skilled nursing facility by providing a nursing facility search engine, combined with ratings per federal quality standards (e.g., Center for Medicare Services), searchable by geographic area and hospital partners.


Impact: With the U.S.’s aging population, there will be more than 78 million people age 65 and over by 2035. As the population ages, the number of people who require skilled nursing facility care grows. There are a limited number of skilled nursing facilities in the U.S. available to serve this population. A simple tool to help patients and families navigate the process of selecting a post-acute care facility will greatly improve the patient experience and make sure patients find the care that’s right for them.


Collaboration: Dr. Thai, Mr. Thai, Mr. Bailey presented their pilot website to Innovation Lab for assistance in taking it to the next level beyond their initial positive results. They collaborated with the Innovation Lab team to validate the unmet need and market problem as well as gauge the size of the opportunity. Innovation Lab enhanced and re-launched the website with improved functionality and an updated look and feel. In addition, they created a strategic business model, including identifying a value proposition, planning for monetization, and developing a horizon-based go-to-market strategy to integrate ad-hoc services and prepare for future expansion beyond skilled nursing facilities.
